That form for the immunisation database?
The one that you have to sign when your child doesn't get vacc'd and you have chosen so?
I heard people calling it a CO form... Where do you get it and what on earth are you supposed to do with the Immunisation records and stuff?
 I am so clueless about it all..

You don't have to do the form, either. It is up to you. Having the form on record, though, might make it easier for you to access childcare and schooling, if that's the path you may take
Once you download the form, you go to your doctor who is obligated to give you the once over about your decision, you say "Yep, I've fully researched the facts and am happy with our decision. Please sign the form now."

No.. The point for me is the maternity allowance. The $200 odd per child for having their records updated enough.
The Dr I will go to with them both (two kids, two forms right?) knows how I feel about the specific one.
I went to the Dr for Baylee's 1y/o shots and the Dr said "No, they are all compulsory on the schedule" when Brett said "That ones voluntary right?"
Which I counteracted with a very annoyed "No, they are ALL voluntary."
The extra $400ish would really make a difference to us.. (where's the embarrassed or sheepish smiley?)
ETA: I had to add, I want neither childcare nor schooling..  Couldn't help it.

Mr Dr actually printed the form up for me which I handed in at medicare. The money came shortly after, though I have heard of peoples forms "going missing" so perhaps keeping a copy of each and calling to follow up may be in order.
From that time onward I have not received the letters I used to get from the immunisation register.

EXCELLENT idea!!! (says she who's form went 'missing', then was miraculously found when I went into a Medicare office & complained about the number of people I knew who's forms went missing)

Just a thought....you could contact the Australian Immunisation Register (which is in WA) phone 1800 653 809 and find out the requirements needed for them (Medicare & Govt) to forward the information onto the Family Assistance Office.
I wouldn't have a clue if you are still entitled to the $$$ but it's worth asking.

Thanks guys.
I've found out that I needed to do Jayden's before he turned 2 to get it. Why they never mention it is beyond me!!
When I went to the Dr to ask for it (Brett talked to him mostly) he was pretty rude and seemed almost amused that we wanted this. He tried his scare tactics and I waited till he was finished to say "I respect that, but no" (in short, anyway).
Turns out it was the wrong form. He seemed very happy with himself that not only did he not know that objectors had forms but that there were indeed objectors!
He filled out the wrong form but it may do the job. If not.. I will be back.
