It might depend where you are to some degree, but really there are no protocols. You'll hit brick walls of ignorance wherever you go. After my HOME lotus birth which required transfer post-partum for sutures the Qld southern zone management committee tried to ban lotus birth in their hospitals

It didn't work. They were arguing infection risk which anyone who knows anything about lotus birth knows is nonsense. And that proved that point didn't it?! I actually found the midwives were fine, and they quickly dubbed the placenta bag "her handbag" in an affectionate way, but the Drs were pains in the bum. I had a few encounters... the paediatrician had a trainee in tow so they both got an ear-bashing from me, then there was the poor youing fella charged with lecturing me about contraception - he got an edumacation too. Bascially, if they were medical then they saw fit to question. Thankfully (?) none of the docs had ever heard of it so they were too dumb-struck to give much back to me but others might not be so lucky.
I have a friend who had a lotus caesarean at Selangor private on the Sunshine Coast. Dr Sarah Buckley believes it may have been the first in Qld. So there must be cooperative obs out there but Selangor breaks all the rules and repoutations so it is probably a one-off.
